A Note to Prospective Patients
It is the mission of Wendi Evans PT to provide quality care that is holistic, comprehensive, and individualized to her patient’s needs. For this reason, she chooses to see only one patient at a time. Initial sessions are 90 minutes, and follow-up sessions are at least 60 minutes in length.
Due to the time-intensive nature of this type of work, Wendi Evans PT is not a participating provider with any insurance companies. However, services may be partially reimbursable under “out of network” physical therapy benefits, depending on the details of the patient’s particular plan. For some plans, patients may be able to submit the receipt on their own. Payment is due at the end of each session. I accept cash, checks, PayPal or credit cards.

Your Right to Receive a Good Faith Estimate of Expected Charges
Under the No Surprises Act
If you are uninsured or have insurance but wish to self-pay, Wendi Evans PT, LLC will provide a “Good Faith Estimate” (“GFE”) of how much our services will likely cost you upon request. If you schedule at least 3 days in advance, we will provide a written GFE within 1 day of scheduling. If you schedule at least 10 days in advance, we will provide a written GFE within 3 business days after scheduling.
We will inform you in advance with a new GFE if we plan to increase our fees or we anticipate your costs exceeding your original GFE. If we don’t and we bill you more than $400 in excess of your GFE without informing you in advance, you will have a right to dispute our bill through a federal dispute resolution process. You will need to provide a copy of your GFE to open a dispute, so we recommend that you save and/or print your GFE for future reference.
